　　請(qǐng)假條格式：

　　標(biāo)題：居中寫(xiě)《請(qǐng)假條》，用來(lái)表明此文是用來(lái)請(qǐng)假的。

　　稱(chēng)呼：平時(shí)怎么稱(chēng)呼，這里就怎么寫(xiě)，后面加冒號(hào)。這個(gè)地方往往是領(lǐng)導(dǎo)、相關(guān)負(fù)責(zé)人等。

　　正文：請(qǐng)假理由。直接寫(xiě)請(qǐng)假的原因，起止時(shí)間。如果有什么事要交代，也可以在寫(xiě)完請(qǐng)假原因之后接著寫(xiě)要交代的事情。正文寫(xiě)完后要加祝頌語(yǔ)，祝福對(duì)方，這個(gè)是所有公文里表示對(duì)對(duì)方的友好。
